ASE turned recurring permit and isolation findings into a system that keeps high-risk work under control, even under production pressure.
The situation
Permit-to-work and lock-out/tag-out existed on paper, but maintenance on live plant kept surfacing the same audit findings — incomplete isolations, confined-space entries without the full control set, and contractor crews working to inconsistent standards.
What ASE did
ASE built a Safety Management System around the high-risk tasks: a disciplined PTW + LOTOTO workflow, HIRA/JSA for each job, contractor safety controls, and competency training for confined-space entry and work at height — held in place by quarterly audits.
